Your loved one's details

As the person has passed away at home, in a nursing home, or in a hospice, please contact us as soon as possible on 0800 652 1830. This will allow us to bring them into our care promptly.
If your plan is with Willow Cremations, please call 0800 652 1830
The death must be verified before we can bring the person who has died into our care. If this hasn't happened yet, still complete this form and we will advise you when we call.
An additional fee will be charged for removal of pacemaker/implants.
I'd like to collect from the crematorium (location decided by Willow Cremations)
I'd like them to be scattered at the Crematorium without me attending (location decided by Willow Cremations)
I'd like to collect them from my local Funeral home (£45.00)
